RCRI (Revised Cardiac Risk Index) considers six risk factors to predicted an increased risk for cardiac complications. A patient's risk for perioperative cardiac complications increases with the number of present risk factors.

To estimate a patient's risk of perioperative cardiac complications.

The RCRI Score is calculated by adding the selected points for each of the six risk factors: High-risk surgery (Intraperitoneal; intrathoracic; suprainguinal vascular) -1p History of ischemic heart disease (History of myocardial infarction (MI); history of positive exercise test; current chest pain considered due to myocardial ischemia; use of nitrate therapy or ECG with pathological Q waves) - 1p History of congestive heart failure (Pulmonary edema, bilateral rales or S3 gallop; paroxysmal nocturnal dyspnea; chest x-ray (CXR) showing pulmonary vascular redistribution) - 1p History of cerebrovascular disease (Prior transient ischemic attack (TIA) or stroke) - 1p Pre-operative treatment with insulin - 1p Pre-operative creatinine >2 mg/dL / 176.8 µmol/L - 1p The risk of major cardiac events (death, myocardial infarction, or cardiac arrest at 30 days after noncardiac surgery) is estimated from the RCRI Score: 0p - 3.9 % risk 1p - 6.0 % risk 2p - 10.1 % risk ≥3 - 15 % risk

Not intended to use for patients younger than 50 year nor in emergency surgery patients, as the revised cardiac risk index was developed from stable patients aged 50 years or more.
