The tool to predict the outcome of steroid therapy in alcoholic hepatitis patient.
To predict response to treatment and survival rate in alcoholic hepatitis patient receiving steroid treatment.
The Lille Model risk can be use to assess the prognosis of patients already receiving steroids for alcoholic hepatitis treatment for 7 days. It is to predict whether the patient will be a good- or non-responder of the steroid treatment. Non-responder patient should be considered for other management strategies. All values besides 7-day bilirubin are taken at time of admission. Lille Model Score = (exp(-R))/(1 + exp(-R)) where the variables are as follows: R = 3.19 - (0.101 * age in years) + (0.147 * albumin in g/L) + (0.0165 * evolution of bilirubin level in µmol/L) - (0.206* renal insufficiency) - (0.0065 * initial bilirubin in µmol/L) - (0.0096 * PT in sec) Evolution of bilirubin level = (Bilirubin day-7) - (Initial bilirubin) Renal insufficiency = 1 (if Cr >1.3 mg/dL (115 µmol/L)) or 0 (if ≤1.3 mg/dL (115 µmol/L)) The Lille Model predicts a 6-month survival rates within 6 months. Scores >0.45 predict a 6-month survival of 25%. Scores <0.45 predict a 6-month survival of 85%.
Not to be used to assess other types of hepatitis. Not to be used to assess patients without steroid treatment.
